Directions:
Once you have downloaded the application form, please save it to your computer and send the completed form to our email address at info@lifteverychild.org. You should receive confirmation of receipt within 48 hours. If you do not hear back from us within that time frame, please send another email or use the "contact us" page on this website.
Please note that you must include a signed copy of the client's treatment plan (or your own, if you are in one of the young adult programs) or good faith estimate from a licensed clinician, as well as a copy of the parent or guardian's (responsible party) state driver's license or other form of identification.
Our grants are ONLY available to those who are most in need. Therefore, as you will see on the application form, we require either proof that the child is covered by Medicaid, OR submission of adjusted gross income from your tax form for the last 2 years of
FEDERAL (not state) tax returns.
We will make every attempt possible to respond to your request within 30 days. Payment will be made directly to the service provider on the client's behalf.

Please understand that although we are eager to support as many neurodivergent individuals as possible, our ability to provide funding depends totally upon the donations we receive. We do not have a source of ongoing financial support, so, unfortunately, the majority of requests will be denied.
